The lowest possible Gleason score is 2 (1 + 1), where both the primary and secondary patterns have a Gleason grade of 1 and therefore when added together their combined sum is 2. For instance, if the Gleason Score is written as 3+4=7, it means most of the tumor is grade 3 and the next largest section of the tumor is grade 4, together they make up the total Gleason Score. If the cancer is almost entirely made up of cells with the same score, the grade for that area is counted twice to calculated the total Gleason Score. As the Gleason scale runs from 1 to 5 it follows that Gleason score results are on the scale of 2 to 10.

Gleason Grade 4 – Majority of glands appear to be interspersed with surrounding tissue. Gleason Patterns As described by Gleason, the grading of prostate carcinoma has to be performed under low magnification (4x or 10x objective). One should not initially use the 20x or 40x objectives to look for rare fused glands or a few individual cells seen only at higher power which would lead to an overdiagnosis of high Gleason patterns. It can be confusing because with the Gleason system (the most common system used), there’s both a “grade” and a “score." We'll explain both for you. The Gleason grade uses numbers 1 to 5. A number is assigned to two of the areas of the prostate that have the most cancer, based on biopsy core samples that are taken. The definition for the new grading system is based on the modified Gleason grading system and incorporates changes made in 2005 and 2014 (see Table 1).

This is a slight change from the pre-2005 Gleason system where the second number was the secondary grade (i.e., the grade of the second-most common cell line pattern). In needle biopsy: The most prevalent pattern is graded as primary and any amount of a worst pattern is graded as secondary. Ex: Gleason pattern 3=96% and pattern 4=4%, Gleason score=3+4=7. Ex: Gleason pattern 3=95% and pattern 4=5%, Gleason score=3+4=7.

Gleason grading system has been accepted as one of the most powerful prognostic indicators in prostate cancer throughout the world. Gleason grading depends solely on architectural patterns of the tumor. !e grade is de#ned as the sum of the two most common grade patterns and reported as the Gleason score. 2021-03-15 · The histological grade (degree of malignancy) of prostate cancer is determined by a pathologist on the basis of a microscopic examination, using the international Gleason grading system. It is based on the histological grade of the glandular structures of prostate cancer tissue or the difference in appearance compared with a normal structure. For prostate cancer, grade group information and Prostate-specific antigen levels are used in conjunction with TNM status to group cases into four overall stages. Grade Group 1 = Gleason 6 (or less) Grade Group 2 = Gleason 3+4=7.

This is a slight change from the pre-2005 Gleason system where the second number was the secondary grade (i.e., the grade of the second-most common cell line pattern). Gleason grading is categorized as Gleason ≤ 6, 3 + 4, 4 + 3, 8, and 9-10, but there is variability within these subgroups. For example, Gleason 4 components may range from 5-45% in a Gleason 3 + 4 = 7 cancer.
